1对夫妻境外输入性寨卡病毒共同感染病例的临床与流行病学特征分析

Clinical and epidemiological characteristics of a couple infected with Zika virus after returning from abroad in Shenzhen, China

摘要: 寨卡病毒病是一种由伊蚊传播的急性病毒性传染病,临床表现以发热、皮疹、结膜炎和关节痛为主。近年来东南亚地区疫情持续活跃,我国虽偶有输入性病例报道,但夫妻共同感染者极为罕见。本文报道了深圳市发现的1对夫妻境外输入性寨卡病毒病共同感染病例,2025年9月27日至10月3日两名患者共同前往马来西亚沙巴州仙本那地区,其间均有明确蚊虫叮咬史,10月4日男性患者首先发病,主要症状为畏寒、高热、全身散在红色斑丘疹及轻度乏力,伴眼结膜轻度充血及关节酸痛。10月5日女性患者相继发病,症状相对较轻,仅表现为低热、轻度头痛,以及颈部、躯干及四肢对称分布的红色斑丘疹。实验室检测显示2例寨卡病毒核酸检测结果均为阳性,登革病毒及基孔肯雅病毒核酸检测结果均为阴性。2例患者经对症支持治疗后,症状于入院后3~5d内明显缓解,均无神经系统并发症。经流行病学调查判定,2例患者为共同暴露型输入性病例。患者所在社区及时开展蚊媒监测与防控,未发现继发病例。总结显示,夫妻共同感染寨卡病毒临床表现存在个体差异,早期识别与干预可减轻病情并有效防止本地传播,为输入性寨卡病毒病的诊治和防控提供参考。

Abstract: Zika virus disease is an acute viral infectious disease transmitted by Aedes mosquitoes, with clinical manifestations primarily including fever, rash, conjunctivitis, and arthralgia. In recent years, epidemics have remained active in Southeast Asia. Although sporadic imported cases have been reported in China, concurrent infection in a married couple is extremely rare. This article reports one pair of imported Zika virus disease cases in a married couple identified in Shenzhen. From September 27 to October 3, 2025, both patients traveled together to Semporna, Sabah, Malaysia, where they had a clear history of mosquito bites. On October 4, the male patient developed symptoms first, mainly presenting with chills, high fever, scattered red maculopapular rash over the body, and mild fatigue, accompanied by mild conjunctival congestion and joint pain. On October 5, the female patient subsequently became ill, with milder symptoms consisting only of low-grade fever, mild headache, and symmetrically distributed red maculopapular rash on the neck, trunk, and limbs. Laboratory testing showed positive Zika virus nucleic acid in both cases, while dengue virus and chikungunya virus nucleic acid tests were negative. After symptomatic and supportive treatment, the symptoms of both patients significantly improved within 3-5 days of hospitalization, with no neurological complications observed. Epidemiological investigation determined that the two cases were co-exposed imported infections. The community where the patients resided promptly implemented mosquito vector surveillance and control measures, and no secondary cases were identified. In summary, clinical manifestations of concurrent Zika virus infection in a married couple can show individual differences. Early recognition and intervention can alleviate the disease severity and effectively prevent local transmission, providing reference for the diagnosis, treatment, and prevention of imported Zika virus disease.
