布鲁氏菌性脊柱炎的MRI研究进展

The research progress of MRI diagnosis of Brucella spondylitis

摘要: 布鲁氏菌性脊柱炎(BS)是由布鲁氏菌感染所致,腰椎发病率最高。患者出现C反应蛋白及红细胞沉降率的升高、虎红平板凝集试验(RBPT)和平板凝集试验(PAT)阳性即可初步诊断为BS。BS临床症状多为腰背部及腰骶部的疼痛、午后低热、多发性骨关节痛等。如果不及时诊断和治疗,将会对脊柱造成严重伤害。BS作为脊柱炎症性疾病的一种,在临床中要注意与脊柱结核、化脓性脊柱炎等其他脊柱疾病的鉴别。MRI具有快速、准确、可重复的特点,对软组织病变具有较高的敏感性,是检出BS首选的影像学检查方法。常规MRI序列结合DTI、IVIM及DCE-MRI等特殊序列,可以在BS的早期诊断、评估疾病进展、判定治疗效果及预后中发挥重要作用,有助于脊柱疾病鉴别诊断,为BS患者临床诊疗过程提供有效参考。

Abstract: Brucella spondylitis is caused by brucella infection, with the highest incidence of the lumbar spine. Patients were initially diagnosed as BS with increased C-reactive protein and erythrocyte sedimentation rate, tiger Red plate agglutination test (RBPT) plate agglutination test and plate agglutination test (PAT) positive.The clinical symptoms of BS are mostly pain in the low back and lumbosacrum, low fever in the afternoon, and multiple osteoarthralgia. If not diagnosed and treated in time, it will cause serious injury to the spine and cause huge economic losses.MR is rapid, accurate and reproducible, has high sensitivity for soft tissue lesions, and is the preferred imaging method for BS detection.Besides,conventional MR sequence combined with special sequences,such as DTI, IVIM and DCE-MRI, can play an important role in the early diagnosis of BS, evaluation of disease progression, determination of treatment effect and prognosis of BS, contribute to the differential diagnosis of spinal diseases, and provide effective reference for the clinical diagnosis and treatment of BS patients.
