新型冠状病毒特异性IgG和IgM抗体检测的假阳性分析

Evaluation of the false positive results of IgG and IgM antibodies to 2019-nCoV

摘要: 目的 分析血清新型冠状病毒特异性IgG和IgM抗体假阳性率及导致抗体假阳性的影响因素,有效发挥抗体检测对新型冠状病毒肺炎的诊断和防控价值。方法 收集2020年8月到2021年2月深圳市第三人民医院9034例住院患者新型冠状病毒核酸检测信息、新型冠状病毒疫苗注射信息及其他相关临床信息;收集患者血浆,采用磁微粒化学发光法检测新型冠状病毒特异性IgG和IgM水平,计算IgG和IgM抗体的假阳性率,统计分析导致新型冠状病毒抗体假阳性的影响因素。结果 ①新型冠状病毒特异性IgG和IgM的假阳性率分别为0.75%和0.34%。②新型冠状病毒特异性IgG(P=0.6098)和IgM(P=0.8288)的假阳性与患者性别无关。新型冠状病毒特异性IgG假阳性与患者年龄无关(P=0.9130),但IgM假阳性受患者年龄影响(P=0.0022),年龄大于45岁人群的IgM假阳性比例明显增加。③新型冠状病毒IgG和IgM抗体假阳性患者中,感染性疾病患者占比最高。结论 磁微粒化学发光法检测新型冠状病毒抗体IgG和IgM的假阳性率较低,年龄大于45岁是新型冠状病毒IgM假阳性的影响因素。

Abstract: Objective To investigate the false positive results of serum IgG and IgM specific to the 2019 novel coronavirus,so as to give full play to the value of antibody detection in diagnosis and prevention and control of COVID-19. Methods The 2019-nCoV nucleic acid detection results, 2019-nCoV vaccine injection history and other relevant clinical information were collected from 9034 hospitalized patients from August 2020 to February 2021. Serum specimens were collected and the levels of IgG and IgM were detected by magnetic particle chemiluminescence. The false positive rate of IgG and IgM and its influencing factors were analyzed. Results ① The false positive rates of IgG and IgM were 0.75% and 0.34%, respectively. ② The false positive results of IgG(P=0.6098)and IgM(P=0.8288) were not correlated with gender. False positive result of IgG was not correlated with age(P=0.9130), while the false positive result of IgM was affected by the age of the patients(P=0.0022). The false positive rate of IgM was significantly increased in the patients with age older than 45. ③In IgG and IgM antibody false positive patients, infected patients accounted for the highest proportion. Conclusion The false positive rates of IgG and IgM detected by magnetic particle chemiluminescence are low. Age over than 45 years is an influencing factor to false positive results of IgM.
