Research progress of multimodal imaging in evaluating HIV infection complicated with cardiovascular disease

摘要: HIV感染作为一种全球范围内的重大公共卫生问题,自发现以来,便对各国医疗卫生系统构成严峻考验。随着抗逆转录病毒治疗(antiretroviral therapy,ART)的广泛应用,HIV感染者的生存周期显著延长,心血管疾病(cardiovascular diseases,CVD)已逐渐成为该人群死亡和致残的主要原因之一。HIV感染合并CVD病理机制复杂、临床表现多样且早期症状不典型,临床诊疗难度较大,因此,早期精准识别与干预尤为关键。多模态影像学整合了超声心动图、心脏计算机断层扫描、心脏磁共振成像、核医学等多种影像技术,打破了单一影像学方法的评估局限,可以从解剖结构、心功能及分子水平进行全面评估。本文围绕多模态影像技术在HIV感染合并CVD疾病中的研究进展进行综述,以期为该病的早期诊断、临床管理及预后评估提供参考。

Abstract: As a major public health problem worldwide, HIV infection has posed a severe test to national health systems since its discovery. With the wide application of antiretroviral therapy (ART), the life cycle of HIV-infected patients has been significantly prolonged, and cardiovascular diseases (CVD) have gradually become one of the main causes of death and disability in this population. The pathological mechanism of HIV infection complicated with CVD is complex, the clinical manifestations are diverse and the early symptoms are not typical. The clinical diagnosis and treatment are difficult, so early accurate identification and intervention are particularly critical. Multimodal imaging integrates echocardiography, cardiac computed tomography, cardiac magnetic resonance, nuclear medicine and other imaging techniques, which breaks the limitations of single imaging method and can be comprehensively evaluated from anatomical structure, cardiac function and molecular level. This article reviews the research progress of multimodal imaging technology in HIV infection complicated with CVD, in order to provide reference for early diagnosis, clinical management and prognosis evaluation of the disease.
