两种新型冠状病毒核酸试剂检测结果的对比研究

Comparison of two state-of-the-art assays for detecting 2019-nCoV nucleic acid

摘要: 目的 比较两种新型冠状病毒核酸试剂检测结果的一致性。方法 回顾性分析深圳市第三人民医院确诊新型冠状病毒患者痰标本、鼻拭子标本和肛拭子标本共计539份核酸检测结果,采用一致性检验进行统计分析。并从两种试剂检测的核酸Ct值均>36的样本中随机抽取4份进行测序验证。结果 两种试剂对痰标本、鼻拭子标本和肛拭子标本一致性检验结果(Kappa值)分别为0.49、0.55和0.71。核酸Ct值<34时,两种试剂在3种标本中阳性一致率均为100%;核酸Ct值≥34时,3种类型标本中2种试剂阳性一致率分别为0、45.0%和57.1%。测序结果证实两种试剂检测核酸Ct值均>36的4份样本中含有新型冠状病毒核酸。结论 两种新型冠状病毒核酸试剂在核酸Ct值较高时(≥34),检测结果一致性较差,使用任一试剂检测都存在假阴性的风险。建议有条件的实验室可使用两种试剂平行检测以提高准确度,未来亟需进一步优化这两种试剂的检测性能。

Abstract: Objective To investigate the consistency of the two assays for the testing of SARS-CoV-2 nucleic acid. Methods 2019-nCoV nucleic acid were parallelly tested in sputum, anal swabs and nasal swabs from COVID-19 confirmed cases by A assays and B assay in the third people's hospital of Shenzhen. 539 results were retrospectively analyzed using consistency test. Four samples with Ct value >36 parallelly tested by two assay were selected for sequencing. Results The Kappa values of nucleic acid results from the two assay were 0.49 in sputum, 0.55 in nasal swabs, and 0.71 in anal swabs, respectively.The consistency rates for the nucleic acid in sputum, nasal swabs, and anal swabs were 100% when the Ct values were <34 parallelly tested by two assay. While, the corresponding consistency rates were 0% in sputum, 45% in nasal swabs, and 57.1% in anal swabs, respectively, when the Ct values were ≥ 34 parallelly tested by two assay. Nucleic acid was tested positively by sequence in the four samples with Ct values >36 parallelly tested by two assay. Conclusions The consistency of nucleic acid results tested by the two assays was low when Ct value was high (≥34), which implies that using single assay of them has the risk of false-negative. Parallelly testing the nucleic acid of 2019-nCoV is an alternative to increase testing accuracy. It is urgent to optimize the efficacy for the two assays.
