机器学习法在艾滋病患者功能磁共振成像中的体素-镜像同伦功能连接价值研究

Investigating value of machine learning in voxel-mirrored homotopic connectivity using functional magnetic resonance imaging for AIDS patients

摘要: 目的 运用基于机器学习法的静息态血氧浓度依赖(blood oxygen level dependent,BOLD)磁共振成像方法,探究体素-镜像同伦功能连接(voxel-mirrored homotopic connectivity,VMHC)在HIV相关神经认知障碍(HIV-associated neurocognitive disorder,HAND)中的诊断价值。方法 收集未进行抗病毒治疗的AIDS患者27例(HIV组)和正常志愿者14例(对照组),以神经心理测评(neuropsychological performance test,NP)为参考标准,将HIV组分为HAND组和非HAND组。经磁共振扫描后所得图像使用DPARSF和PRoNTo软件行后处理。结果 VMHC指标在HIV组与对照组差异贡献最大的10个大脑区域依次是:左中央旁小叶、右中央旁小叶、右辅助运动区、右楔前叶、左楔前叶、右顶下缘角回、左辅助运动区、左尾状核、右中央后回、左颞横回。VMHC指标对区别HAND组及非HAND组没有意义。双辅助运动区、右顶下缘角回、左尾状核、右中央后回、左颞横回的VMHC值与部分临床量表、认知评分有不同程度的相关性。结论 VMHC值为临床诊断HAND提供客观的标准,对降低HAND的发生和发展可发挥一定的作用。

Abstract: Objective To explore the diagnostic value of voxel-mirrored homotopic connectivity(VMHC) values in the assessment of HIV-related neurocognitive disorders using magnetic resonance resting state functional imaging based on machine learning. Methods The twenty seven AIDS patients without antiviral therapy (HIV group) and 14 normal volunteers (control group) were included. Taking the neuropsychological performance(NP) test as a reference standard, the HIV patients were grouped into the HIV-associated neurocognitive disorder(HAND ),sed using DPARSF and PRoNTo software. Results The ten brain regions in which the VMHC index contributed the most difference between the AIDS patients and the control group included the bilateral paracentral lobule, the right supplementary motor area, the bilateral precuneus,the right inferior parietal gyrus, the left supplementary motor area, the left caudate nucleus, the right postcentral lobule, the left heschl gyrus. VMHC index was no significant between HAND group and non-HAND group. The VMHC values of the bilateral supplementary motor area,the right inferior parietal gyrus, the left caudate nucleus, the right postcentral lobule, the left heschl gyrus were related to clinical scales and cognitive scores. Conclusion VMHC value is a useful marker for HAND, which may contribute to the occurrence and development of HAND.
