2023年深圳地区182例手足口病和疱疹性咽峡炎患儿肠道病毒优势血清型VP1基因特征分析

VP1 gene characteristics of predominant enterovirus serotype of 182 hand, foot, and mouth disease and herpangina patients from Shenzhen in 2023

摘要: 目的 调查2023年深圳地区手足口病(hand, foot, and mouth disease,HFMD)/疱疹性咽峡炎(herpangina,HA)患儿肠道病毒血清型分布情况,分析优势血清型的进化特征。方法 采集2023年深圳市宝安区妇幼保健院和深圳市妇幼保健院182例HFMD/HA患儿粪便或咽拭子样本,利用荧光定量PCR方法检测肠道病毒,阳性样本利用PCR-基因测序法分型,根据分型结果得出优势血清型,利用PCR-基因测序法获得优势血清型的VP1基因编码区序列全长,再利用MEGA X软件构建系统发育进化树,利用Bioedit软件进行序列相似性分析。结果 182份样本肠道病毒检出率为62.1%(113/182),优势血清型为柯萨奇病毒A组6型(Coxsakie virus A6,CV-A6),系统发育进化树显示CV-A6可分为4个谱系(A~D),2023年深圳分离株均属于谱系D中的D3a亚支,与谱系A~C代表株的核苷酸、氨基酸相似性分别为82.0%~85.1%、93.7%~96.7%。结论 CV-A6是2023年深圳地区HFMD/HA患儿肠道病毒的优势血清型,其基因分型属于当前全球流行的D3a亚支。

Abstract: Objective To investigate enterovirus serotypes in children with hand, foot, and mouth disease (HFMD) or herpangina (HA) in Shenzhen during 2023, and analyze evolutionary traits of the predominant serotype. Method Fecal sample or throat swabs, collected from 182 HFMD/HA patients in Shenzhen during 2023. Enteroviruses were detected by quantitative real-time PCR, and positive samples were typed via PCR-sequencing; besides, the full-length VP1 sequences of the predominant serotype of enterovirus were obtained for phylogenetic tree construction using MEGA X, and sequence similarity analysis using Bioedit. Result Enterovirus prevalence was 62.1% (113/182), with coxsackievirus A6 (CV-A6) as the predominant serotype. Phylogenetic analysis resolved CV-A6 into 4 lineages (A-D),with all 2023 Shenzhen isolates assigned to the D3a subclade of linages D. Nucleotides and amino acid sequence identities with representative strains of lineages A-C were 82.0%-85.1% and 93.7%-96.7%, respectively. Conclusion CV-A6 was the predominant enterovirus serotype in 2023 HFMD/HA pediatric cases from Shenzhen, belonging to the globally circulating D3a subclade of lineage D.
