AIDS合并结痂性疥疮并发败血症1例

Acquired immune deficiency syndrome combined with Norwegian scabies complicating sepsis: a case report

摘要: 疥疮是由疥螨引起的接触性传染性皮肤病,结痂性疥疮是疥疮的特殊类型,主要表现为广泛的皮肤干燥、脱屑、结痂、裂缝等。因症状与其他皮肤疾病相似,容易混淆,极易误诊,导致患者治疗不及时或接受不必要的治疗。AIDS患者免疫功能受损会增加结痂性疥疮发病风险,使其更具侵袭性,导致预后不良。本文回顾性分析重庆市公共卫生医疗救治中心收治的1例AIDS患者感染结痂性疥疮并发败血症的病例资料,该患者最初被误诊为银屑病,对症治疗无效后通过皮肤病理组织活检明确诊断为结痂性疥疮,同时结合辅助检查,考虑合并败血症。提示当AIDS患者出现广泛的皮肤炎症性改变时,需要警惕结痂性疥疮的可能,应当尽早行病理学检查,明确诊断,及时治疗。

Abstract: Scabies is a contact infectious skin disease caused by scabies mites. Norwegian scabies is a special type of scabies, also known as scabies. It is mainly characterized by extensive skin dryness, desquamation, scabs, cracks, etc. As the symptoms are similar to other skin diseases, it is easy to be confused and misdiagnosed, resulting in patients not receiving timely treatment or receiving unnecessary treatment. HIV infection impairs cell-mediated immunity, increases the risk of Norwegian scabies and makes more aggressive, leading to a poor prognosis. In this paper, we retrospectively analyzed the data of a patient with acquired immunodeficiency syndrome infected with Norwegian scabies complicated with sepsis admitted to Chongqing Public Health Medical Treatment Center. This patient was initially misdiagnosed with psoriasis, after symptomatic treatment failed, the Norwegian scabies was clearly diagnosed by skin pathological biopsy, and sepsis was considered combined with auxiliary examination. Suggests that when AIDS patients have extensive skin inflammatory changes, it is necessary to be alert to the possibility of Norwegian scabies, and pathologic examination should be performed as soon as possible to make a clear diagnosis and timely treatment.
