CT仿真内镜对艾滋病合并支气管结核的临床应用价值

Diagnosis value of CTVE in AIDS combined with bronchial tuberculosis: a comparative analysis with bronchoscopy

摘要: 目的 探讨CT仿真内镜( computed tomography virtual endoscop,CTVE )对艾滋病合并支气管结核的临床应用价值。方法 收集2013年1月至2019年7月深圳市第三人民医院确诊为艾滋病且临床表现符合支气管结核的43例病例,行CT薄层连续容积扫描并用CT横断图像及CTVE图像对支气管病灶进行评估,以支气管镜检查结果为参照进行对比分析。结果 在支气管病灶的检出率方面,CT横断面图像和CTVE图像分别为62.8%( 27/43)和83.7%(36/43),两者差异有统计学意义(P=0.021);CTVE与支气管镜检查对比,两者差异无统计学意义(P=0.375)。结论 CTVE对支气管结核有较高的诊断符合率,是一种非侵袭性的检查方式,易于被艾滋病患者接受,具有较高的临床应用价值和广阔的临床应用前景。

Abstract: Objective To explore clinical value of CT virtual endoscopy(CTVE)imaging in the diagnosis of AIDS combined with bronchial tuberculosis,by contrast analysis with bronchoscopy. Methods Forty-three patients with confirmed AIDS and clinical manifestations with bronchial tuberculosis underwent CT thin-slice continuous volume scan, CT traversal images and CTVE images were used to evaluate bronchial lesions, and compared with bronchoscopy, the examination results were analyzed by statistical methods. Results For the detection rate of bronchial tuberculosis, traversal images and CTVE images were 62.8%(27/43) and 83.7%(36/43), respectively (P=0.021), and the difference was statistically significant. Compared with bronchoscopy, CTVE had no statistical significance (P>0.375). Conclusions CTVE has a higher diagnostic accuracy for bronchial tuberculosis, and it is also a non-invasive examination method, which is easy to be accepted by AIDS patients, and high clinical application value and broad clinical application prospects.
