Construction of "micro-ward" teaching guided by post competence in the standardized training for resident physicians in department of infectious diseases based on negative pressure ward

摘要: 目的 探讨在负压病房中构建以岗位胜任力为导向的“微病房”教学并将其应用于感染科住院医师规范化培训(standardized training for resident physicians,STFRP)中的效果。方法 采用试验对照方法,将2024年8月至2025年2月于上海长海医院感染科参加STFRP的115名学员随机入组为研究组(n=56)和对照组(n=59),研究组采用以岗位胜任力为导向的“微病房”教学模式,对照组采用传统教学模式,通过理论考核、技能考核、病例分析、岗位胜任力评价和问卷调查评估学习效果。结果 研究组学员总成绩、理论考核、技能考核、病例分析和岗位胜任力评价分数均高于对照组(P<0.05),其中理论考核中的多项选择题,技能考核的流畅度,病例分析中临床判断能力、治疗决策能力、人文关怀素养、整体胜任能力等具体项目的成绩差异有统计学意义(P<0.05)。问卷调查显示,研究组的“促进协同合作、提高学科兴趣、增加理论收获”3个条目的综合态度系数F值最高。两组进行比较,研究组9个条目的F值均高于对照组,其中“加强自主学习、鼓舞临床热情、激发科研兴趣”的F值差异最大。结论 在传染科负压病房中将以岗位胜任力为导向的“微病房”教学应用于STFRP中可有效提升学员培训结束的考核成绩,提高学员对传染病的关注度,有效促进协同合作。

Abstract: Objective To explore the construction of "micro-ward" teaching guided by post competence in negative pressure wards and the effect of it in the standardized training for resident doctors(residential training) in department of infectious diseases. Method A total of 115 trainees who participated in residential training in the department of infectious diseases of Changhai Hospital from August 2024 to February 2025 were randomly assigned to the study group (n=56) and the control group (n=59). The study group adopted the "micro-ward" teaching mode guided by post competency, while the control group adopted the traditional teaching mode. The learning effect was evaluated through theoretical examination, skill examination, case analysis, post competency evaluation and questionnaire survey. Result The total score, theoretical assessment, skill assessment, case analysis and post competency assessment scores of the study group were higher than those of the control group (P<0.05). Among them, the scores of multiple-choice questions in theory assessment, fluency in skill assessment, clinical judgment ability, treatment decision-making ability, humanistic care literacy, overall competence and other specific items in case analysis had statistical significance (P<0.05). The questionnaire survey showed that F value of the comprehensive attitude coefficient of the study group in three items of "promoting collaborative cooperation, improving subject interest and increasing theoretical gain" was highest. Compared between the two groups, F values of 9 items in the study group were higher than those in the control group, among which F values of "strengthening independent learning, encouraging clinical enthusiasm and stimulating scientific research interest" had the greatest difference. Conclusion In the negative pressure ward of department of infectious diseases, the application of "micro-ward" teaching guided by post competence in residential training can effectively improve the assessment results of students at the end of training, improve students' attention to infectious diseases, and effectively promote cooperation.
