重症百日咳预后相关影响因素分析

摘要: 百日咳是由百日咳鲍特菌感染引起的高度传染性呼吸道疾病。近年来,包括中国在内的许多百日咳疫苗接种覆盖率较高的国家陆续出现“百日咳再现”,给公共卫生安全带来了巨大的挑战,引发了国内外广泛关注。2024年中国百日咳发病人数激增,死亡例数创历史新高。同时,由于儿童、青少年和成年人感染百日咳后早期症状与其他上呼吸道感染难以区分,常常被误诊或漏诊。百日咳在中国的发病率可能被严重低估。重症百日咳由于常伴严重并发症,甚至有导致死亡的风险。早期识别高危患者有助于医务人员及时采取针对性干预措施,改善患者预后、降低并发症发生率与病死率、提升医疗质量和效率、减轻因重症百日咳造成的家庭经济负担和社会公共卫生资源的消耗。本文综述了与重症百日咳预后相关的影响因素,旨在为临床快速识别重症患者提供决策支持,同时为临床诊疗方案的优化和新型预防疫苗的研发提供依据,为未来同类研究提供参考。

Abstract: Pertussis is a highly contagious respiratory disease caused by Bordetella pertussis infection. In recent years, many countries with high pertussis vaccination coverage, including China, have experienced the “re-emergence of pertussis”, posing a significant challenge to public health. This has caused widespread concern both domestically and internationally. Pertussis cases surged, and deaths hit a record high in China in 2024. Meanwhile, pertussis infection in children, adolescents and adults are often misdiagnosed or underdiagnosed due to the difficulty in distinguishing them from other upper respiratory tract infections during the early symptoms of the infection. In fact, the morbidity of pertussis in China may be seriously underestimated. Severe pertussis poses a serious threat to life and health, as it carries a risk of death due to the severe complications that commonly accompany the disease. Early identification of high-risk patients can help healthcare professionals implement timely and targeted interventions to improve patient prognosis, reduce the incidence of complications and case fatality rates, enhance the quality and efficiency of healthcare, and lessen the economic burden on families as well as the consumption of public health resources caused by severe pertussis. This article reviews the influencing factors associated with the prognosis of severe pertussis, aiming to provide decision support for the rapid clinical identification of critically ill patients, as well as to provide a basis for the optimization of clinical diagnostic and treatment protocols and the development of novel preventive vaccines, and to provide a reference for similar studies in the future.
