小儿难治性肺炎支原体肺炎血清自分泌运动因子、白介素-6、白介素-33表达水平及临床意义

Expression levels and clinical significance of serum autotaxin, IL-6, and IL-33 in children with refractory Mycoplasma pneumoniae pneumonia

摘要: 目的 分析小儿难治性肺炎支原体肺炎(refractory Mycoplasma pneumoniae pneumonia,RMPP)血清自分泌运动因子(autotaxin)、白介素-6(interleukin-6,IL-6)、白介素-33(interleukin-33,IL-33)表达水平,探究三者对评估RMPP病情转化风险的有效性,为临床控制普通肺炎支原体肺炎(general Mycoplasma pneumoniae pneumonia,GMPP)进展为RMPP提供参考。方法 选取2022年1月1日至2024年1月1日彭州市妇幼保健院急诊科收治的83例RMPP患儿及80例GMPP患儿为研究对象,收集患儿人口学、临床资料进行回顾性分析。对比GMPP组、RMPP组一般资料,分析autotaxin、IL-6、IL-33与肺功能指标相关性以及GMPP进展为RMPP的危险因素。结果 GMPP组、RMPP组性别、年龄、BMI、白细胞计数比较,差异无统计学意义(均P>0.05);GMPP组发热时间≤10d、无胸腔积液、累及范围单肺叶的占比较RMPP组低,RMPP组肺实变区CT值、autotaxin、IL-6、IL-33水平均高于GMPP组,潮气量(tidal volume,VT)、到达呼气峰流速的时间与总呼气时间比(time to peak tidal expiratory flow as a proportion of expiratory time, tPTEF/TE)、呼出75%潮气量时的瞬间流速与潮气呼气峰流速比(TEF25/PTEF%)值低于GMPP组(均P<0.05)。Logistic回归分析结果显示:发热时间(>10d)、胸腔积液(少量、中等/大量)、肺实变区CT值高、累及范围(多肺叶),以及autotaxin、IL-6、IL-33、VT、tPTEF/TE、TEF25/PTEF%水平上升是影响GMPP进展为RMPP的独立危险因素(均P<0.05)。autotaxin、IL-6、IL-33与VT、tPTEF/TE、TEF25/PTEF%均存在负相关性(均P<0.05)。autotaxin、IL-6、IL-33联合检测对GMPP进展为RMPP预测敏感度、特异度分别为92.80%、88.20%,大于三者单独检测(P<0.05)。结论 autotaxin,IL-6,IL-33在RMPP患儿血清中的表达水平显著升高,与患者肺功能密切相关,有望作为预测儿童GMPP进展为RMPP的生物学指标。

Abstract: Objective To analyze the expression levels of serum autotaxin, interleukin-6 (IL-6) and interleukin-33 (IL-33) in children with refractory Mycoplasma pneumoniae pneumonia (RMPP), and to explore their effectiveness in evaluating the risk of transformation from general Mycoplasma pneumoniae pneumonia(GMPP) to refractory RMPP. In order to provide reference information for clinical control of GMPP progression in RMPP. Method A total of 83 children with RMPP and 80 children with GMPP admitted to the Emergency Department of Pengzhou Maternal and Child Health Care Hospital from January 1, 2022 to January 1, 2024 were selected as the research objects, and their demographic and clinical data were collected for retrospective analysis. The correlation between autotaxin, IL-6, and IL-33 with pulmonary function indicators and the risk factors affecting the progression of GMPP to RMPP were analyzed by comparing the general data of GMPP and RMPP groups. Result There were no significant differences in gender, age, BMI, and white blood cell count between GMPP group and RMPP group (P>0.05). Compared with the RMPP group, the GMPP group had a lower proportion of patients with duration of fever ≤10 days, no pleural effusion, and involvement of ≤ single lung lobe, and the RMPP group had higher CT value in the consolidation area and higher levels of autotaxin, IL-6, and IL-33. Tidal volume(VT), time to peak tidal expiratory flow as a proportion of expiratory time(tPTEF/TE) and TEF25/PTEF% in the GMPP group were significantly lower than those in the GMPP group (P<0.05).Logistic regression analysis showed that: duration of fever (>10 days), pleural effusion (small, moderate/large), high CT value of consolidation area, involvement range (multiple lung lobes), increased autotaxin, IL-6, IL-33, VT, tPTEF/TE, TEF25/PTEF% levels were independent risk factors for the progression of GMPP to RMPP (P<0.05). Autotaxin, IL-6 and IL-33 were negatively correlated with VT, tPTEF/TE and TEF25/PTEF% (all P<0.05).The sensitivity and specificity of autotaxin, IL-6, and IL-33 combined detection in predicting the progression of GMPP to RMPP were 92.80% and 88.20%, respectively, which were higher than those of the three alone (P<0.05). Conclusion The expression levels of autotaxin, IL-6, and IL-33 in the serum of children with refractory Mycoplasma pneumoniae pneumonia were significantly increased, which were closely related to the lung function of the patients. They are expected to be used as biological indicators for predicting the progression of GMPP to RMPP in children.
