广西首例人感染H10N3禽流感合并重度急性呼吸窘迫综合征患者静脉-静脉体外膜肺氧合治疗策略探析

摘要: 目的 探讨静脉-静脉体外膜肺氧合(veno-venous extracorporeal membrane oxygenation,VV-ECMO)在治疗人感染H10N3禽流感并发重度急性呼吸窘迫综合征(acute respiratory distress syndrome,ARDS)患者中的应用价值及可行性。方法 回顾性分析2024年12月南宁市第四人民医院收治的广西首例人感染H10N3禽流感并发ARDS患者应用VV-ECMO辅助治疗过程及效果。结果 患者主要临床表现为发热、咳嗽及气促。经中国疾病预防控制中心对患者呼吸道样本的流感病毒深度测序确诊为人感染H10N3禽流感。患者在接受VV-ECMO支持治疗后,肺部病灶得以吸收,最终痊愈。结论 VV-ECMO在人感染H10N3禽流感并发ARDS患者中的应用展现了显著的治疗成效,为疾病控制及肺功能恢复赢得了宝贵时间,可提升患者的生存率。

Abstract: Objective To explore the application value and feasibility of veno-venous extracorporeal membrane oxygenation (VV-ECMO) in the treatment of patients infected with H10N3 avian influenza complicated with severe acute respiratory distress syndrome (ARDS). Method A retrospective analysis was performed on the process and effect of VV-ECMO adjuvant treatment in the first case of human infection with H10N3 avian influenza complicated with ARDS in Guangxi. Result The main clinical manifestations of the patient were fever, cough and shortness of breath. The patient was confirmed to be infected with H10N3 avian influenza by the Chinese Center for Disease Control and Prevention (China CDC) through deep sequencing of influenza virus in respiratory samples. After receiving VV-ECMO supportive treatment, the pulmonary lesions were absorbed, and the patient ultimately recovered. Conclusion The application of VV-ECMO in patients infection with H10N3 avian influenza complicated with ARDS has shown remarkable therapeutic effects. It can efficiently provide oxygenation and remove carbon dioxide, winning precious time for disease control and pulmonary function recovery, and significantly improving patient survival rates.
