摘要: 新中国自成立以来便不断完善重点传染病的监测工作,2003年SARS疫情之后建立了横向到边、纵向到底的传染病与突发公共卫生事件网络直报系统。随着病原体传播影响因素不断变化,对监测预警关口、预警信息、预警技术要求的不断提高,以及分子生物学、生物信息学、人工智能等鉴定分析技术的不断发展,增强早期监测预警能力逐渐成为健全公共卫生体系的当务之急。本共识组织中国医学科学院、广东省疾病预防控制中心、深圳市疾病预防控制中心、《新发传染病电子杂志》编辑部、深圳市动物疫病防控中心、成都市疾病预防控制中心等16家机构共计31位相关领域知名专家,通过对当前重点传染病(39种法定传染病和新发传染病)形势总结分析,基于“同一健康”理念,针对重点传染病,形成第四代综合监测方案。旨在通过对人、动物、病媒生物等主要疫源宿主和相关环境的病原体,结合流行传播影响因素、风险评估要素,展开流行病学和病原学系统性监测,以早期发现、预测预警重点传染病的发生、发展、变异和流行传播特征。同时,基于第四代综合监测方案,我们尝试构建传染病综合监测组织管理体系、数据融通共享的数智化监测预警平台和集约化病原检测技术体系。这一系列的思考,以多部门、多维度、多因素、多病原的系统性、综合性监测为基础,以数据融合、互通互联为支撑,以多领域合作为契机,以多学科结合应用为依托,为全面提升重点传染病综合监测预测预警能力提供参考,供传染性疾病预防控制和相关机构参考使用。

Abstract: China has continuously improved the monitoring of key infectious diseases. After the SARS epidemic in 2003, we have established a comprehensive reporting network system for infectious diseases and public health emergencies. With the constant development of molecular biology, bioinformatics, artificial intelligence, and other identification and analysis technologies, along with the increasing demands for early warning thresholds, information, and techniques, strengthening early monitoring and warning capabilities has become a top priority for enhancing the public health system. A total of 31 experts, based on the "One Health" concept and through analysis of the current situation, have suggested the fourth-generation comprehensive surveillance system targeting key infectious diseases (39 notifiable diseases and emerging infectious diseases). This surveillance system aims to systematically monitor the epidemiology and pathogenics of major disease hosts such as humans, animals, and vectors, along with the associated environmental pathogens. By integrating factors influencing epidemic spread and risk assessment elements, the surveillance system aims to detect, predict, and provide early warnings for the occurrence, development, variation, and spread of new and key infectious diseases. Moreover, we are attempting to build an organizational management system for comprehensive infectious disease monitoring based on the fourth-generation surveillance system, along with a data-integrated monitoring and early warning platform and an intensive pathogen detection technology system. This series of considerations, based on systematic and comprehensive monitoring across multiple sectors, dimensions, factors, and pathogens, is supported by data integration and connectivity. It provides an opportunity for collaboration in various fields and relies on interdisciplinary application to enhance the comprehensive monitoring, prediction, and early warning capabilities for the next generation of infectious diseases surveillance. This serves as a reference for infectious disease prevention and control and related organizations.
