儿童下呼吸道肺炎支原体感染流行特点分析

Epidemiological characteristics analysis of lower respiratory Mycoplasma pneumoniae infection in children

摘要: 目的 探讨儿童下呼吸道肺炎支原体感染(MP)流行特点,以期为MP感染防治工作提供数据资料。方法 收集2017年1月至2019年12月湖北省妇幼保健院收治的1134例下呼吸道感染(LRI)患儿的临床资料。采取回顾性研究的方法,制定统一的临床研究表,对纳入患儿的临床资料进行逐份详细阅读,收集汇总相关信息,包括性别、年龄、入院时间、临床症状及MP抗体检测结果。结果 1134例LRI患儿中,155例检出MP阳性,MP构成比为13.67%;其中,2017、2018、2019年的MP构成比分别为14.40%、11.17%、15.49%,经比较差异无统计学意义(P>0.05)。男、女患儿的MP构成比分别为13.11%(78/595)、14.29%(77/539),差异无统计学意义(P>0.05)。MP构成比随着年龄的增长逐步增高,其中0~3岁LRI患儿的MP构成比最低,为10.26%,其次为4~5岁的15.41%、6~14岁的16.44%;经比较,0~3岁LRI患儿的MP构成比显著低于4~5岁和6~14岁LRI患儿的MP构成比,差异有统计学意义(P<0.05)。全年均可出现MP感染,以10–; 12月的MP构成比最高,为17.21%,其次为1–; 3月的14.01%、4–; 6月的10.67%、7–; 9月的9.28%;经比较,10–; 12月的MP构成比显著高于4–; 6月和7–; 9月的MP构成比,差异有统计学意义(P<0.05)。结论 4~14岁儿童为MP易感人群,患儿主要表现为咳嗽、发热、喘息,且该病在秋冬季节更易高发。

Abstract: Objective To explore the epidemiological characteristics of lower respiratory Mycoplasma pneumoniae (MP) infection in children so as to provide data for the prevention and treatment of MP infection in children. Methods The clinical data of 1134 children with lower respiratory infection (LRI) in Hubei Maternal and Child Health Hospital from January 2017 to December 2019 were collected. Based on retrospective research method, the unified clinical research forms were formulated to read the clinical data of children in detail. The related data were collected and summarized, including gender, age, admission time, clinical symptoms and test results of MP antibody. Results Among the 1134 LRI children from 2017-2019, there were 155 cases (13.67%) with positive MP. The ratios of MP infection in 2017, 2018 and 2019 were 14.40%, 11.17% and 15.49%, respectively. And the difference was not statistically significant (P>0.05). The ratios of MP infection in males and females were 13.11% (78/595) and 14.29% (77/539), and the difference was not statistically significant between the groups (P>0.05). The ratio of MP infection was gradually increased with age. The ratio of MP infection was the lowest in LRI children aged 0-3 years (10.26%), followed by 4-5 years old (15.41%) and 6-14 years old (16.44%). After comparison, ratio of MP infection in LRI children aged 0-3 years was significantly lower than that aged 4-5 years old and 6-14 years old (P<0.05). There was MP infection throughout the year, with the highest ratio of MP infection within October-December (17.21%), followed by January-March (14.01%), April-June (10.67%) and July-September (9.28%). After comparison, ratio of MP infection within October-December was significantly higher than that within April-June and July-September (P<0.05). Conclusion Children aged 4-14 years old are the susceptible population. The main symptoms include cough, fever and wheezing. And the disease is more likely to appear in autumn and winter.
