Severe dengue with platelet membrane glycoprotein antibody positive: report of 1 case LIN Lu-ping, FAN Hui-min, LI Yue-ping, CHEN Xie-jie, ZHAO Ling-zhai, YAO Xi-an, YE Xin, DENG Jing, TAN Xing-hua, ZHANG Fu-chun 2018, 3 (2):  95-97.  Abstract ( 257)    PDF (1160KB) ( 627 )    Objective To understand the clinical features of severe dengue with platelet membrane glycoprotein antibody positive and to explore its clinical management. Methods The clinical data and treatment of a severe dengue fever case with positive platelet membrane glycoprotein antibody in our hospital were retrospectively analyzed. Results The 21-year old, male patient,with acute onset manifested one-day fever and systemic rash. 6 days after fever, the patient was found gingival bleeding and urine bleeding. The regular blood examination showed that HGB：151.00g/L, PLT：2.00×109/L, HCT：43.3%. After infusion of platelets, platelet recovery was not obvious and the index of platelet dropped to 1.00×109/L. Dengue fever antibody was found positive.The main treatment measures including absolute bed, acid inhibition and other comprehensive treatment. Platelet relative antibody were examined. The antibody GPIIb/Ⅲa was positive. Given the gamma globulin and hormone, the patient's platelets rebounded, the bleeding symptoms remission, and the platelets rebounded. Conclusion Severe Dengue could lead to severe bleeding.Platelet relative antibodies should be checked. Gamma globulin and glucocorticoid might be helpful. The timing of usage and doseneed further study.

Immune responses induced by dengue serotype Ⅱ virus DNA vaccine are not associated with gender in mice WANG Ran, GAO Na, AN Jing 2018, 3 (2):  98-101.  Abstract ( 249)    PDF (992KB) ( 594 )    Objective To investigate the difference in protective efficacy of dengue serotype Ⅱ virus DNA vaccine pVAX1-D2ME between male and female mice. Methods Male and female mice were separately immunized with pVAX1-D2ME plasmids. Sera were obtained before each immunization and after the last immunization to evaluate the humoral immune responses. Two weeks after the last immunization, the mice were challenged with the dengue serotype Ⅱ virus (DENV2) to observe the difference of survival rate between male and female mice. Results Male and female mice showed no significant differences in the levels of specific antibodies produced by this vaccine. The immune response showed a Th1-type tendency. All the mice immunized showed complete protection against the lethal dose of virus with 100% survival rate. Conclusion sThe pVAX1-D2ME could induce strong immune response and effective protection against DENV2 in mice, and these effects are not related to the gender of mice.

Kinetics of dengue virus IgM/IgG antibodies and factor analysis in patients with dengue ZHAO Ling-zhai, YU Lei, HONG Wen-xin, QIU Shuang, YANG Hui-qin, WANG Jian, ZHANG Fu-chun 2018, 3 (2):  102-105.  Abstract ( 354)    PDF (1727KB) ( 768 )    Objective To investigate the kinetics of dengue virus IgM/IgG antibodies and to analyze the relationship between immune status and severity of dengue. Methods Dengue virus IgM-specific and IgG-specific antibodies in 1371 patients with dengue were detected using ELISA. Based on their levels, the primary or secondary infection was differentiated. Results The IgM antibody increased gradually with progression of the disease and the positive rate increased up to 95% at d 6 after onset of the disease. The IgG antibody increased more slowly. The incidences of primary and secondary infections of dengue were 75.02% and 24.98%, respectively. The incidence of secondary infection showed no significant difference between patients with mild and severe conditions. But the incidence of secondary infection was significantly higher in the elderly patients (52.79%) than the young and middle aged adult patients (18.39%). Conclusion sMost patients with dengue in China are diagnosed with primary infection. Higher incidence of secondary infection is observed in the elderly patients.

CT manifestations of clonorchiasis and its complications LIU Hong-shan, FANG Jing, LIAO Jin-yuan 2018, 3 (2):  106-110.  Abstract ( 296)    PDF (3826KB) ( 733 )    Objective To investigate the radiological manifestations of clonorchiasis and its complications. Methods A total of 1567 cases with definitive diagnosis of clonorchiasis were retrospectively analyzed. The CT and MRI manifestations of liver cirrhosis, cholangiectasis, bile duct stones, biliary tract infection, as well as hepatobiliary malignancies were observed and analyzed. Results In the total 1567 cases of clonorchiasis, there were 144 cases of acute infection and 1102 cases of chronic infection. And the other 321 cases showed negative by CT and MRI. The cases with acute infection are manifested as biliary tract infection. And the cases with chronic infection are manifested as biliary tract stones, simplex cholangiectasis, and hepatobiliary malignancies. Conclusion The radiology demonstrates acute stage of clonorchiasis commonly as pericholangitis, which may be complicated by mild dilation of intrahepatic bile duct. And the radiology demonstrates chronic stage of clonorchiasis as bile duct stones as well as slight or moderate dilation of extrahepatic peripheral bile duct, which may be complicated by malignancies. Radiology has important value in detecting and differentiating the complications of clonorchiasis.

CT scanning of mono-resistant pulmonary tuberculosis during initial treatment CHEN Gen-ming, CHENG Guan-xun, ZHU Shao-qian, ZENG Ling-yan, WEN Zhao-yi, ZHUANG Yan-ni 2018, 3 (2):  111-114.  Abstract ( 265)    PDF (2040KB) ( 671 )    Objective To explore the CT manifestations and characteristics of mono-resistant pulmonary tuberculosis in the primary therapy,in order to raise awareness of mono-resistant pulmonary tuberculosis among population. Methods 30 patients with mono-resistant pulmonary in the primary therapy from Baoan Chronic Diseases Prevent and Cure Hospital between July 2015 and December 2017 were treatedas the mono-resistant group, and 30 patients with sensitive to anti tuberculosis drugs in the primary therapy during the same period were selected as the control group. The difference in age,gender,the distribution of lesions and CT manifestations in two groups were analyzed by statistical software. Results Chi square test showed that there was no significant difference in gender and disease distribution between the single drug resistance group and the control group (P>0.05),The age of single drug resistance group was smaller than that of the control group (P<0.05). The incidence of single drug resistance in large lobar or segment type, atelectasis, non-wall cavity and bronchial thickening was significantly higher than that of the control group (P<0.05), and the incidence of lymph node calcification in the mediastinal mediastinum was significantly lower than that of the control group (P<0.05). The incidence of calcification in mediastinal lymph nodes was significantly lower than that in the control group (P<0.05). Multifactor logistic regression analysis showed that large lobar or segmental consolidation, non-wall cavity, bronchial thickening and stenosis,non-calcification of hilum and mediastinal lymph nodes were the main risk factors for mono-resistant pulmonary tuberculosis in the primary therapy. Conclusion TheCT manifestations of mono-resistant pulmonary tuberculosis in the primary therapy are mainly active and extensive. When there are large lobar or segmental consolidation, non-wall cavity, bronchial thickening stenosis and non-calcification of hilum and mediastinal lymph nodes, it will be helpful for the diagnosis of mono-resistant pulmonary tuberculosis in the primary therapy.

Differentiation of benign and malignant pulmonary nodules by computer-aided diagnosis of digital chest X-ray ZHAO Wen-li, GUO Hong-yun, LIN Tu-xing, QUAN Shen-wen, LI Mu-lan 2018, 3 (2):  115-117.  Abstract ( 271)    PDF (1584KB) ( 612 )    Objective To observe and analyze differentiation of benign and malignant pulmonary nodules by computer-aided diagnosis(CAD) of digital chest radiography. Methods A total of 154 patients with singular nodule treated in our hospital between January 2016 and June 2017 were recruited as subjects for the retrospective analysis. The subjects include 80 patients with malignant nodule and 74 with benign nodule. All malignant pulmonary nodules were confirmed by pathology, while all benign pulmonary nodules were confirmed by CT scans and their reading by more than 2 radiologists. All chest X-ray films were obtained from the Digital Radiography (DR) system, which were read by 5 senior radiologists and 5 junior radiologists. The chest X-ray films with and without CAD were analyzed. The analysis was performed using the Receiver Operating Characteristic (ROC) curve for observers’ scores. Results The average area under the ROC curve (AUC) increased gradually from 0.713 (without CAD) to 0.792 (with CAD) (P<0.05) in junior radiologists group. But senior radiologists group showed no significant difference (P>0.05). The risk scores of AUC output from CAD was positively correlated with the malignant nodules. Therefore, the enhancement of AUC showed significant differences. Conclusion CAD of digital chest radiography can more effectively facilitate junior radiologists diagnosing and differentiating benign and malignant pulmonary nodules in patients with singular nodule. The patients with early accurate diagnosis thus can receive timely appropriate treatment. Therefore, CAD of digital chest radiography deserves wide promotion in clinical practice.
